Endowment Bequest*:
“I give and bequeath to Texas Children's Hospital, located in Houston, Texas (Tax ID No. 74-1100555), [ __________ ] to establish (or add to) the [Name of individual] Endowed Fund, in support of ________________.”
Contingent Bequest:
“I give and bequeath to my spouse, if he or she survives me, [ ___________ ]. If my spouse does not survive me, I give and bequeath to Texas Children's Hospital, located in Houston, Texas (Tax ID No. 74-1100555), [ ___________ ] to be used for its general purposes (or other purpose*).”
Honorary or Memorial Bequest:
